Form of Faith

Election to membership occurs when a candidate has passed all parts of the exam and when a Form of Faith has been received, verified and processed satisfactorily. Until this process has been completed doctors are not considered elected to membership. The Form of Faith is retained as a permanent record of a candidate’s agreement to abide by the laws, bye-laws, statutes and regulations of the Royal Colleges of Physicians of the United Kingdom.

How to complete the Form of Faith

Candidates are required to complete the form with their own details and ensure this is dated where specified.

Candidate are encouraged to submit their completed form electronically to their College of entry via email addresses provided on the form.

Candidates are strongly advised to submit their completed Form of Faith once it has been confirmed they have passed the final part of the examination. Any delay in submission of a completed Form of Faith will result in a delay in the candidate being elected to MRCP(UK), obtaining their MRCP(UK) diploma and admission to the ceremony of their choice.
